RESUMO
Com a elaboração do presente documento, pretende-se espelhar o percurso realizado no sentido da aprendizagem, aquisição e desenvolvimento de competências de Mestre e Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica.
O ambiente de cuidados em que decorre o parto e o pós-parto imediato tem profundas implicações no estabelecimento do início precoce da amamentação. Amamentar o recém-nascido durante a primeira hora de vida dá-lhe melhores hipóteses de sobreviver, crescer e desenvolver todo o seu potencial. O Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica encontra-se numa posição privilegiada para promover a fisiologia do parto e cuidados respeitosos após o nascimento, equilibrando o protagonismo da mulher/família e facilitando o início precoce da amamentação, aumentado a sua probabilidade de sucesso e a qualidade dos cuidados em Enfermagem de Saúde Materna e Obstétrica.
O ciclo reprodutivo, gravídico e puerperal pauta-se pela constante necessidade de adaptação da mulher/casal/família aos estímulos físicos, emocionais, psicológicos e sociais que encerra, pelo que, o percurso formativo aqui demonstrado foi norteado pela Teoria de Grande Alcance de Callista Roy, no seu Modelo Adaptativo.
Metodologicamente, optou-se pela realização de uma Scoping Review, segundo as orientações do Joanna Briggs Institute Manual for Evidence Synthesis, com objetivo de mapear a evidência científica existente acerca das intervenções do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica para a promoção do início precoce da amamentação, durante o contacto pele-a-pele. Foi, ainda, realizado um estudo primário, qualitativo, exploratório e descritivo, visando identificar e descrever as intervenções do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica para a promoção do início precoce da amamentação, assim como explorar a sua perceção acerca das implicações destas intervenções nos cuidados prestados.
Os resultados obtidos sugerem que o parto eutócico e a realização do contacto pele-a-pele imediato estão estreitamente relacionados com o início precoce da amamentação, seguidos de práticas de cuidados como a presença da pessoa significativa, a manutenção da privacidade e a realização de procedimentos durante o contacto pele-a-pele. As intervenções do Enfermeiro Especialista em Enfermagem de Saúde Materna e Obstétrica passam pela promoção da fisiologia do parto e pós-parto imediato, facilitação do contacto pele-a-pele ininterrupto e independentemente da via do parto, manutenção das condições ambientais favoráveis e apoio emocional e/ou físico à mulher, até à concretização da primeira mamada espontânea do RN.
The purpose of this document is to reflect the path taken towards learning, acquiring and developing the skills of a master’s degree and Nurse-Midwife. The way in which childbirth and the immediate postpartum period take place has profound implications for establishing the early initiation of breastfeeding. Breastfeeding the newborn during the first hour of life gives them the best chance of surviving, growing and developing to their full potential. The Nurse-Midwife is in a privileged position to promote the physiology of childbirth and respectful care after birth, balancing the role of the woman/family, thus facilitating the early initiation of breastfeeding, increasing its likelihood of success and the quality of care in Maternal and Obstetric Health Nursing. The reproductive, pregnancy and puerperal cycle is characterized by the constant need for women/couples/families to adapt to the physical, emotional, psychological and social changes it entails, and so the training path shown here was guided by Callista Roy's Wide-Range Theory, in her Adaptive Model. Methodologically, we opted to conduct a Scoping Review, according to the guidelines of the Joanna Briggs Institute Manual for Evidence Synthesis, with the purpose of mapping the existing scientific evidence on the interventions of Nurse-Midwife to promote the early initiation of breastfeeding during skin-to-skin contact. A primary, qualitative, exploratory and descriptive study was also carried out to identify and describe the interventions of Nurse-Midwife to promote the early initiation of breastfeeding, as well as to explore their perception of the importance of these interventions. The results obtained suggest that the eutocic delivery and immediate skin-to-skin contact are closely related to the early breastfeeding initiation, followed by care practices such as the presence of the significant companion, maintaining privacy and carrying out the procedures during skin-to-skin contact. The Nurse-Midwife's interventions include promoting the physiology of childbirth and the immediate postpartum period, facilitating immediate, uninterrupted skin-to-skin contact, regardless of the delivery mode, maintaining favourable environmental conditions and providing emotional and/or physical support to the woman until the NB's spontaneous first feed.
